An "!" icon appears instead of uploaded thumbnail, why?

The image might have been corrupted during processing or the original RAW file is corrupted.

If the image was corrupted during processing, please clear the library using the “clear” button, delete the newly created optimized files that are corrupted, reimport your original image(s) and start the processing again.
